Look closely at your insurance card. The small logos on the front and back of the card identify your insurance carrier.

Insurance carriers we directly participate with include (may differ for each individual doctor):

  • BlueCross BlueShield PPO, POS, HMO
  • Aetna Health Care
  • United Health Care
  • Cigna Health Care PPO, POS, HMO
  • PHCS (Private Health Care Systems)

NOTE that PHCS (Private Health Care Systems) has a large umbrella, which cover many companies not named above. If an insurance carrier does not directly participate with us, you may still have benefits which cover our services. Call your insurance carrier to see if you must meet a deductible.

Regardless of how you think your coverage will respond, you should call your insurance and ask the representative for both your in- and out-of-network chiropractic benefits. Most carriers carry benefit information on their website, see the back of your insurance card for information.

Georgia Sports Chiropractic Cancellation Policy

Out of respect and consideration to your Chiropractor, we kindly ask that you honor your scheduled appointment time. Please note that appointment space is limited daily and appointments are booked back to back.

Georgia Sports Chiropractic understands that unanticipated events occasionally do happen in everyone’s life.  In our desire to be effective and fair to all of our clients’ time, we kindly ask that you give at least a 24 hour advance notice within normal office hours or leave a voice message on the machine when cancelling and/or changing an appointment.  This allows the opportunity for someone else to schedule an appointment.

Chiropractic / Personal Insurance / Work Comp Cancellations/Changes/No-Shows with less than 24 hours notice when canceling or rescheduling appointments may be subject to partial or full charge for that appointment.   It is office policy that you are allowed two ( 2 ) missed appointments or cancellations with no penalty. After the second cancellation or missed appointment, you will be charged the full amount of your chiropractic adjustment and/or other therapy treatment fees. Insurance can not and will not be billed for these charges. Cancellation fees are the responsibility of the patient and must be paid in full before the next visit.
